Parish National Bank Visa Gift Card Terms
& Conditions
These are your Parish National
Bank Visa Gift Card Terms & Conditions.
In these Terms & Condition “you” or “your” means the person who received
the Parish National Bank Visa Gift Card; “we”, “us”, or “our” means Parish
National Bank located in Covington, La.;
“Card” means the Parish National Bank Visa Gift Card. Please read the following carefully and keep
them for your records. Please sign your
Card immediately. By accepting and using
your Card, you agree to these Terms & Conditions.
Lost/Stolen Card or Unauthorized Use
Though you may not be liable
for the unauthorized use of your Card, you must notify us of the loss, theft or
possible unauthorized use as soon as it is known to you in order to limit any
potential loss. If you believe your Card
has been lost or stolen, call or write to us at the telephone number and
address listed as follows: 1-800-201-3534, 24 hours a day, 7 days a week, or
write to: Parish National Bank, Card Services Department, P. O. Box 1470,
Covington, La 70433.
A replacement card or check will be issued for the value remaining on
the Card at the time it is reported lost or stolen and the original card will
be canceled. There is a $10.00 service
charge for the replacement card. If the
remaining balance on the card is less than $35.00, a check will be issued instead of a new card and you will be charged a $15.00
check issuance fee.
Card Activation
Before using your Parish
National Bank Visa Gift Card, you must activate it with your personal
identification number (PIN) by visiting www.parishnational.com or by calling 1-800-675-1001.
You must sign the back of the card before
you can use the Card.
Card Usage
Your Parish National Bank Visa
Gift Card may be used for purchases at merchants where Visa debit cards are
accepted. Your daily point-of-sale limit
will not exceed $1,000.00.
The use of your Card to
purchase goods and services from merchants and at ATM locations constitutes a
simultaneous demand against and withdrawal from the value stored on your card.
This Gift Card is not a credit
card. You may only use it up to the
remaining balance on the card. Purchases
will be deducted from your Card until the value reaches zero. If the purchase exceeds the balance on the
card you must pay the difference. If you are given a value through the use of
the Card greater than the balance remaining, you will pay us on demand the
amount by which your transactions exceeded the balance remaining on the Card. There is a fee of $29.00 per excess use if
the Card is used in excess of amount stored on the Card.
The Card and Pin are provided
for your use and protection, and you will:

Not disclose the
Pin nor record it on the Card or otherwise make it available to anyone else;
·
Use the Card, the
Pin and ATM as instructed;
·
Promptly notify us
of any loss or theft of your Card or Pin;
·
Be liable for any
transaction made by a person you authorize or permit to use your Card or Pin or
both. If you authorize someone else to
use your Card or Pin, you will be responsible for any transactions initiated by
such person with your Card or Pin; and
·
Be responsible for
all authorized transactions initiated by use of your Card.
If
you have a problem with any goods or services you purchase using the Card, you
agree to settle any disputes arising from the problem directly with the
merchant who provided the goods or services.
Cash Transactions
You are permitted to use the
Card to access cash at an ATM with your Personal Identification Number
(PIN). There is an ATM/Cash Access fee
of $2.50 per ATM transactions regardless of which institution owns the
ATM. (When you use an ATM not owned by
us, you may be charged an additional fee by the ATM operator or any network
used to complete the transfer.) You may obtain
up to $500.00 per day in cash from ATM that displays any of the logos appearing
on your Card.
Reloadable or Non-reloadable
If your Parish National Bank
Visa Gift Card is personalized with your name on it, it may be reloaded by
visiting www.parishnational.com or by calling 1-800-201-3534.
If your Card is an Instant
Issue (not personalized with your name) it is non-reloadable and cannot be
reused.
Monthly Maintenance Fee
There will be a $2.00 monthly
maintenance fee deducted from the balance remaining after 6 months from
issue. After 12 months from issue the
monthly maintenance fee will increase to $5.00 per month. For balance inquiries call 1-800-201-3534.
Transaction Restrictions
Your Card may not be used for International
transactions.
You may not make
pre-authorized regular payments through the use of your Card.
The Card can be used at gas stations;
however, at gas stations with a “pay at the pump” capability, the card cannot
be authorized directly at the pump.
Simply present the Card to the attendant for payment.
If your purchase amount is
greater than the amount available on your Card, the difference can be paid with
another form or payment, depending on the policy of the merchant. You must know the amount available on your
Card and inform the merchant to process the transaction in that amount. If you
are given a value through the use of the Card greater than the balance
remaining, you will pay us on demand the amount by which your transactions
exceeded the balance remaining on the Card.
There is a fee of $29.00 per excess use if the Card is used in excess of
amount stored on the Card.
Personalized Cards are nontransferable. The cardholder is the person whose name is
embossed on the front of the card and is the only person authorized to use the
card.
You may use your Card only in
the manner and for the purposes authorized by these Terms &
Conditions. You do not have the right to
stop payment on any point of any point-of-sale transaction originated by use of
your Card.
Replacement Cards
A replacement card can be
issued for the value remaining on any damaged, expired, lost, or stolen Card
and the original card will be canceled by 1-800-201-3534. There is a $10.00 Card Replacement fee for
issuing a new card.
Duration of Card
Your Card is valid until the
“Good Thru” date shown on the front of your Card. We encourage you to use the entire gift
amount before this date. Any
transactions that are attempted after Good Thru date will be declined. Upon your request we will issue a check for
the balance remaining on your expired Card minus a $15.00 Check Issuance Fee.
Errors and Problems
If you think there is an error
involving a transaction made with your Card such as a purchase you didn’t make
or an incorrect amount charged to your Card, please call us at 1-800-240-3271

We must hear from you within
60 days. You will need to provide:

Your name and PNB
Visa Gift Card number
·
The dollar amount
of the suspected error and approximately when the error took place
·
Describe the error
and explain, if you can, why you believe there is an
error. If you need more information
describe the item you are not sure about.
If you tell us orally, we will
require that you send your complaint or question in writing within 10 business
days. Generally, we will tell you the
results within 10 business days after we hear from you and will correct any
error promptly. If we need more time,
however, we may take up to 45 calendar days to investigate your complaint or
question. If we decide to do this, we
will re-credit your Card within 10 business days for the amount you think is in
error so that you will have the use of the money during the time it takes us to
complete our investigation. If we do not
receive your complaint or request in writing within 10 business days, we may
not re-credit your Card. If notice of an
error involves a point–of-sale debit, or occurred within 30 days after the Card
was issued, the applicable time period for completing an investigation will be
90 calendar days in place of the 45 calendar days. A written explanation will be sent to you
within 3 business days after the investigation of the alleged error has been
completed. You may ask for copies of
documents that we used in our investigation.
Your Liability For A Lost/Stolen Card or
Unauthorized Transfers
Telephoning us at the phone
number listed under “Lost/Stolen Card or Unauthorized Use” section above is the best way of keeping your possible losses
down. You may also write us at the
address listed in “Lost/Stolen Card or Unauthorized Use” above. If you tell us within 2 business days after
you discover the loss or theft of your Card, you can lose no more than $50.00
if someone used your Card without your permission. If you do not tell us within 2 business days
after learning of the loss or theft, you may be liable
for up to $500.00 if someone used your Card without your permission.
Cancellation
Parish National Bank retains
the right to amend or discontinue the Parish National Bank Visa Gift Card
program and these Terms & Conditions at any time at our discretion. The Parish National Bank Visa Gift Card will
remain our property. We may cancel your
rights to use the Card at any time and have you return the Card to us. There is a check issuance fee of $15.00 to
close the Card and receive a check for the value remaining on the Card.
Other Terms & Conditions
Parish National Bank will not
be liable to you for failure to honor the Card.
The Parish National Bank Visa
Gift Card is not an FDIC insured deposit.
You must notify us immediately
if your address has changed. To update
your information by using the Gift Card link on our website, www.parishnational.com or you may
contact us in writing at the address listed below. Your letter should include the new address,
phone number, your name, and Card number:
